*Please only submit this form if you are a current UW-Madison student who is not assigned an AAS advisor and would like to be assigned to our office*

• If you have declared an L&S major, please see your major advisor, visit our drop-in advising, or submit our advising appointment request form.
• If you are not currently in L&S, please see our Transfer to L&S page.
• If you are assigned to Cross-College Advising Services (CCAS), please see your CCAS advisor.
• If you are a new incoming student, please do not submit this form. You will be assigned an academic advisor when you attend SOAR.
